Directly scraped endometrial cells

  • Better preserved cellular morphology
  • Hyperchromatic crowded cell groups
  • Large sheets, may be thick
  • Broad ribbons of glandular cells with regular outline
  • Biphasic pattern
  • Not uncommonly found in smears with vigorous scraping
  • Post-LEEP or post-cone smears
  • Thick large fragments - Cell block may help

  Glandular cells

  • Dark  nuclei +/- overlapping
  • Somewhat coarse or granular chromatin
  • +/- Distinct nucleoli
  • Cytoplasm variable, (proliferative phase: scanty cytoplasm, Secretory phase: more clear cytoplasm)
  • Mitosis may be seen during  proliferative phase

Stromal cells

  • Spindle, fusiform  or ovoid
  •  In isolation or surrounding sheets of glandular cells
  • At proliferative phase: scanty cytoplasm +/-  mitosis
  • At late secretory phase: predecidual cells, more cytoplasm, more cohesive
